Support Services

The goal of Support Services is to coordinate a comprehensive continuum of care that encompasses social support necessary to assist clients in obtaining an optimum of care. This can be done by providing, coordinating, and assuring social support necessary to assist clients in maintaining their prospective health, safety, and well-being. Some direct care services are, but are not limited to:

  • Vision and Dental Services
  • Behavioral Health Referrals
  • Food Assistance (Nutrition Assistance / Mobile Market Services)
  • Resource Center Services
  • Insurance Assistance
  • Short-Term Rental / Mortgage / Utility Assistance
  • Housing Opportunity for People Living with HIV/AIDS (HOPWA) Program
  • Transportation Assistance
  • Housing Referral Assistance
  • Medical Case Management
  • Support Groups (Women’s HIV Group, CARES Support Group, and Rural Support Group)
